PATIENT EXPERIENCE

Quality, access, and reliability all affect an individual patient’s experience. TCA aims to improve the patient experience by working to transform care across the state of Texas. By creating patient-centered models of care, providers can ensure that the best and most appropriate care is given to individuals and families, thus enhancing the overall experience of care.

POPULATION HEALTH

As a primary focus of TCA, population health management encourages providers, communities and patients to work together to improve the health status of entire populations. By analyzing data across a large number of patients, providers are better able to target and treat patients with chronic disease, as well as identify healthy patients for wellness initiatives. These insights can reduce health disparities, and lead to new models of care delivery focused on prevention and wellness.

PER CAPITA COSTS

Healthcare reform aims to reduce the cost of healthcare per person. TCA strives to lower per capita costs by facilitating collaboration and coordination of care, sharing best practices to improve the treatment and prevention of chronic disease, and encouraging a patient-centered, clinician-led approach to personal health management.
